Periodontal status is determined by the doctor and may require a plan upgrade. The advanced periodontal therapy plan is for patients who are in need of a deep cleaning. This plan would be purchased one time. At the time of renewal, after one year of treatment, the patient can choose the periodontal care membership.
Terms and Conditions
12-month membership fee is due in full upfront.
Membership is effective immediately.
It is the sole responsibility of the member to maximize their benefits by arranging all the the appropriate appointments within the 12-month membership period. If appointments are not used, the member will not be entitled to a refund.
Renewal payment is due after 12 months on the last day of the month. There is no auto draft, so payment must be scheduled.
The anniversary month of membership is the month you originally joined, regardless of when payment is received.
When CareCredit® is applicable, services will be reduced by 5%.
Treatments prior to joining do not qualify for member discounts.
Zoom (in-office whitening) is not covered.
15% reduced fees DO NOT include cosmetic services.
If the member has any failed or missed appointments with 48 hours, the contract will be terminated.

Simply schedule an appointment with our office and be sure to tell us you’re a member. When you arrive, we’ll make sure your membership is current and you will be eligible for instant savings off our regular fees.

Can I transfer my membership to another dentist?
Priority Care Membership is NOT a dental insurance plan; it can only be used at Vitality Dental. Membership is non-transferable.

Can I combine a membership with other dental coverage?
No, membership cannot be combined with other coverage.
